To add to that, Tomko was spotted in the Pittsburgh airport by reader Scott Hensley with his arm in a sling. Tomko mentioned that he may have broken something in his arm, probably during the RAW dark match. Below is the photo that Scott sent along:

Not really that daft, I just read the article and that's what it sounded like. Out of shape generally = fat, or at least not super muscly.

He wasn't fat until the last few months of his stay in TNA, so the coming off of steroids theory is pretty sound here. Tomko it's possible would be much the same.
